Where our story began
Active Health was founded in 2023 by a physical therapist with decades of experience in the field. After years of caring for patients in home health agencies, skilled nursing facilities, assisted living facilities, board and care homes, and beyond, the need for high-quality care and timely communication became clear. Active Health Services has been raising the bar ever since.
